No provider-patient relationship
Visiting this website, downloading a referral form, or emailing the office does not by itself create a provider-patient relationship. Acceptance of any referral, case, or lien-based arrangement depends on clinical review, documentation, and administrative approval.
Email and electronic communication
Email may not be secure unless a secure transmission method is arranged. Please do not send sensitive medical records by unsecured email unless an appropriate authorization and secure transmission method are in place. Our office can provide next steps for records transfer when needed.
Records and billing requests
Records, billing, or case-related requests may require a signed authorization or other supporting documentation before our office can release or discuss protected health information. Please email the office with the appropriate request, and our office will respond with next steps.
Emergencies
Do not use this website or email for emergencies. If you are experiencing a medical emergency, call 911 or go to the nearest emergency department.
Referral and lien-related review
Lien-based arrangements, case acceptance, and treatment planning are reviewed on a case-by-case basis. Clinical decisions and documentation are based solely on medical judgment.
